The University of Washington Medical Center has an outstanding opportunity for a full-time, day shift, SOCIAL WORKER 2 - MENTAL HEALTH INTEGRATION PROJECT CARE COORDINATOR. The Mental Health Integration Project (MHIP) care coordinator functions as a core member of the Outpatient Psychiatry (OPC) collaborative care team that involves a consulting psychiatrist and other members of the clinic interdisciplinary care team. This position supports the care team by ensuring that the mental health needs of the patient are met, including ongoing assessment of psychosocial functioning, coordinating referrals to clinically indicated services, consulting with, and serving as a liaison with psychiatry, and case managing the patient to ensure increased adherence to both the primary care and mental health treatment.

 

RESPONSIBILITIES:

 

  • Support and closely coordinate mental health care with the patient’s psychiatrist, primary care provider and the interdisciplinary team 
  • Screen and assess patients for common mental and substance abuse disorders 
  • Provide patient education about common mental and substance abuse disorders and available treatment options 
  • Monitor clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications 
  • Support psychotropic medication management, focusing on treatment adherence, attention to side effects, and effectiveness of treatment 
  • Provide brief counseling or psychotherapy using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills 
  • Provide or facilitate in-system or external referrals to evidence-based psychosocial treatments (e.g, CBT, IPT, PST) as clinically indicated, as well as other social services (e.g., housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment) 
  • Conduct regularly scheduled (usually weekly) caseload consultation with the consulting team psychiatrist, and communicate resulting treatment recommendations to the patient’s PCP 
  • Track patient follow-up and clinical outcomes using the web-based MHITS care management tracking system 
  • Document in person and telephone encounters in MHITS and use the system to track patient progress, treatment recommendations, and to identify and reengage patients who may be lost to follow-up 
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP and the team psychiatrist 
  • Facilitate GA-U/GA-X / SSI eligibility process with CSO 
  • Complete relapse prevention plans with clients who are in remission. 
  • Complete core curriculum and participate in regular training and technical assistance activities for MHIP Care Coordinators

REQUIREMENTS:


A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND two years of full-time professional social work experience in a health-care setting eligible to begin social work practicum instruction for the School of Social Work.
OR
Equivalent education/experience.

 

LEGAL REQUIREMENT:

  • Within 90 days of hire must hold one of the following credentials issued by the state of Washington: 
  • Licensed Advance Social Worker 
  • Licensed Independent Clinical Social Worker

 

DEPARTMENT REQUIREMENTS:

 

  • Care coordinators shall be qualified as a Mental Health Professional under WAC 388-865-0100, or qualified as a Mental Health Care Provider and supervised by a Mental Health Professional or MSW 
  • Experience with screening and assessment for common mental/substance use disorders, including a working knowledge of differential diagnosis of common mental/substance use disorders and evidence-based psychosocial treatments 
  • Familiarity with brief, structured counseling techniques (e.g., Motivational Interviewing, Behavioral Activation) 
  • Basic knowledge of psychopharmacology for common mental disorders

 

DESIRED:

 

  • Experience working with underserved, transient populations 
  • Experience working with patients who have co-occurring mental health, substance abuse, and physical health problems 
  • Experience with evidence-based counseling techniques (e.g. CBT or PST) 
  • Experience working with safety net providers within the community and knowledge of community resources 
  • Experience working with, and understanding requirements of DSHS eligibility programs, including GA-U, GA-X, and SSI 
  • Ability to work with CSO (Community Service Office) and CHP staff and GA-U client to maintain/obtain GA-U/GA-X/SSI benefits 
  • Working knowledge of the RSN mental health system structure and regulations
